How To ObtainYou can purchase a certified birth record for anyone born in Minnesota in 1935 or after and a certified death record for anyone who died in Minnesota 1997 or after at the Government Center Vital Statistics office or at any Anoka County License Center. To obtain a certified record for births recorded in Anoka County before 1935 or deaths recorded in Anoka County before 1997, you will need to go to the Government Center Vital Statistics office. Required FormsDownload the Minnesota Birth and Death Record application form (Adobe Acrobat format), type in your information and print it, or... Pick up the application form at the Government Center Vital Statistics office or at any Anoka County License Center, or... Request that the application form be sent to you. In some cases, you may need someone to fill out the Statement to Identify Certified Birth or Death Certificate Applicant form (Adobe Acrobat format). If submitting an order by mail or fax, download and print the Vital Statistics Credit Card Form (Adobe Acrobat format) Required IdentificationThe following documents must be accepted as meeting the requirements, unless the document has been altered or changed, or there is evidence of fraud on the face of the document.
Required Fees (effective 8-1-05)$16.00 First Certified Birth Record $9.00 Each additional Birth Record of the same record issued at the same time. $13.00 First Certified Death Record without cause of death (only for records 1997 to present). $13.00 First Certified Death Record with cause of death. $6.00 Each additional Death Record of the same record issued at the same time. Anoka County License Centers can accept payment by cash or check.
