How To Obtain
Please note - Birth/death record applications will be accepted at the Vitals Statistics Office and the Columbia Heights, Coon Rapids, and Ham Lake License Centers but not at the Blaine and Ramsey License Centers. See License Center Hours and Services for more information.
You can purchase a certified birth record for anyone born in Minnesota in 1935 or after at the Government Center Vital Statistics office or at the Columbia Heights, Coon Rapids, and Ham Lake License Centers.
In order to obtain a certified birth record, you must show that you have a tangible interest in the record.
By mail or fax:
For birth records, download the Minnesota Birth Record application form (Adobe Acrobat format). Print it and fill it out.
Request that the application form be sent to you.
In some cases, you may need someone to fill out the Statement to Identify Certified Birth or Death Certificate Applicant form (Adobe Acrobat format).
If submitting an order by mail or fax, download and print the Vital Statistics Credit Card Form (Adobe Acrobat format)
The following documents must be accepted as meeting the requirements, unless the document has been altered or changed, or there is evidence of fraud on the face of the document.
$26.00 First Certified Birth Record (effective July 1, 2010)
$19.00 Each additional Birth Record of the same record issued at the same time (effective July 1, 2010)
$13.00 First Certified Death Record without cause of death (only for records 1997 to present).
$13.00 First Certified Death Record with cause of death.
$6.00 Each additional Death Record of the same record issued at the same time.
Anoka County License Centers can accept payment by cash or check.
If submitting an order by mail or fax, you can pay with VISA or MasterCard by using the Vital Statistics Credit Card Form (Adobe Acrobat format)
