Driver's Instruction PermitFor information about other licenses and services, see the License Bureau home page How To ObtainA Driver's Instruction Permit can be obtained, in person, at any Anoka County License Center. Submit a valid test score sheet and proof of identity, or current permit. If replacing a lost or stolen permit, proof of identity must be presented. A vision test will be given. Applicants under 18 years of age must have the application notarized by parent or guardian. Note: Anoka County License Centers do NOT offer written or "behind-the-wheel" driver's license testing. See the Minnesota Department of Vehicle Services Web site for testing locations. Required FormsThe Driver's Instruction Permit application form is available in any Anoka County License Center. First-time applicants must have a valid test score sheet.
