Liquor License

This information applies only to Anoka County townships: Burns, Columbus, and Linwood.

How To Obtain

Fill out all three pages of the Liquor License application form and the Certification of Compliance form.
The application form must also be signed by a Notary Public, a service available at the Government Center Vital Statistics office or any Anoka County License Center.
Then, either:
- Submit the forms in person, with required fee, at the Government Center Vital Statistics office, or
- Mail the forms and required fee to:
  
  Anoka County Government Center
  Vital Statistics
  325 East Main Street, W120
  Anoka, MN 55303
- This license cannot be obtained at Anoka County License Centers.

You can display and print Adobe Acrobat files with the free Reader program, which can be downloaded from the Adobe Web site.

Download the Liquor License application form (Adobe Acrobat format), type in your information and print it (remember that the signature must be notarized), and...

Download the Certification of Compliance form (Adobe Acrobat format), type in your information and print it, or...

Pick up the application and certification forms at the Government Center Vital Statistics office, or...

Request that the application and certification forms be sent to you.

Required Identification

Notarization requires a Driver's License or State Identification Card.

Required Fees

Temp Beer Event - $50
Temp Liquor Event - $75

The Government Center Vital Statistics office can accept payment by cash, check or credit card (Mastercard and VISA).
