Hospice care is a concept of care. It involves a special system of caring for patients, usually in their own home, during their final months. A hospice team provides medical and nursing care to patients in conjunction with their personal physician and family. This quality, end-of-life compassionate care is for someone who has a progressive, life-limiting illness and is no longer receiving treatment toward a cure. A public health nurse can put seniors and family members in touch with service providers or refer them to other community resources for help.
