Finding Long-term Care

Seniors may have health care needs that require more than visits to a clinic or a short stay in a hospital. Medical care may be needed over a long period of time and may be delivered through home-based care, assisted living or a nursing home.

Anoka County public health nurses play a key role in assessing what kind of long-term health care is best for a senior's situation.  For example, a preadmission screening by a public health nurse is a requirement before admission to a nursing home.
