Anoka County Preadmission Screenings by a Public Health Nurse For more information or to set up a preadmission screening, call The long-term care consultation is an evaluation done at the senior’s home by a public health nurse. The purpose of the consultation is to assess the health care and medical needs of the senior. The result of the consultation is a comprehensive set of recommendations the senior and family members can consider for maintaining the health and care of an individual.
