Adult Foster Care - Homes Plus Program

For more information on Homes Plus (adult foster care), call
Anoka County Human Services
763-422-7070

Anoka County arranges care for low-income seniors through a program called “Homes Plus.” Homes Plus housing is licensed by the Minnesota Department of Human Services.

Individuals or families in licensed homes provide foster care, or care can be provided by a business entity that staffs its home with paid employees who care for seniors. Adult foster care homes for seniors provide five basic services: room, food, supervision, protection, and household (for example, washing clothes). In addition, adult foster care homes help seniors with dressing, personal hygiene, eating, taking medication, money management and transportation (to appointments, senior centers, shopping and other activities).

Adult foster care might be 24-hours a day, 7-days a week (for an indefinite period of time), respite care, or day care. Before an individual can be placed in an adult foster care home, a social worker and a public health nurse must complete an assessment.
