Long-Term Care Consultation (LTCC)
Long-Term Care Consultations (LTCC) are done by a county public health nurse
or a social worker who will visit with you in your home or in a
long-term care facility to help you identify services you might need
or want. They will help you plan those services.
The LTCC visit is free of charge.
LTCC consultants can help you:
- Stay in your own home or apartment
- Choose services you prefer
- Move from a long-term care facility
- Move to foster care or housing with services
- Understand the choices available to you
- Develop a plan to meet your needs
- Access programs that help pay for services if you are eligible
Keep in mind that:
- When requested, the LTCC team provides:
- information and education about long-term care options
- an in-person visit to assess needs and help plan services
- information about public programs that can help pay for
services
- transition assistance for people who want to return to the
community after admission to a hospital or nursing home
- There is no charge for an initial assessment and support
planning visit, whether provided in the community, to avoid a move
to a nursing home, or as a transition to return to the community
- The LTCC team will make recommendations for community services
or a nursing facility when community-based services are not feasible
- The resident is included in the final decision about services
